Initial Consultation

At your first visit with Connecticut Health and Wellness, we take the time to get to know you as a whole person, not just your symptoms. Your provider will review your medical history, lifestyle, nutrition, stress, sleep, and any current concerns you would like to address. With your consent, a focused assessment may be performed as well as a review of any recent lab work, imaging, or other test results you may have.

Together, we will develop a personalized care plan designed to address the root causes of your health concerns. This may include nutritional and lifestyle recommendations, functional lab testing, supplementation, or prescriptions when appropriate. Referrals to specialists or additional therapies may also be discussed based on your needs.

Because every patient is unique, the number of visits and type of follow-up will vary depending on the complexity and duration of your health concerns. Our goal is to partner with you in restoring balance, supporting long-term wellness, and helping you feel your best.

Patients often report leaving the first visit with a clearer understanding of their health and a sense of empowerment.

Cancellation Policy

At Connecticut Health and Wellness, we value both your time and ours. To ensure we can best serve all of our patients, we kindly ask that any appointment changes or cancellations be made at least 24 hours in advance. Appointments canceled with less than 24 hours’ notice, or missed without notice, will be subject to a $75 cancellation fee. This policy helps us reserve time for patients who are waiting to be seen and maintain the highest level of care for everyone in our practice.

Patient Forms

All required forms are conveniently completed through the Practice Better patient portal. Please ensure your forms are submitted prior to your scheduled visit so we can make the most of your appointment time.
